General Terms

Education, Programmes, and Limitations of Professional/Medical Advice

It's important for you to understand that WellOnTrack Service(s) and Programme(s) are of a general nature and should not replace any current medical care you may be receiving. Whilst I share and embrace your enthusiasm for Lifestyle Medicine, if you require more personalised advice, please consult your preferred health professional. I’m your Health Coach and NOT a Medical Professional and, I don’t provide or offer professional medical evaluation, advice, diagnosis or treatment ("Medical Advice"), nor a replacement for such medical advice. WellOnTrack provides information only. I may provide you with access to general health education and information, nutritional and fitness and other lifestyle advice, and references to other resources, according to the information you supply to or request from me. While I believe that the information I provide is current, reliable and accurate, WellOnTrack makes no such guarantee or warranty.

Before you make use of any WellOnTrack Service or information, you hereby agree that you will consult a suitably qualified physician or other healthcare professional for any medical advice, particularly if changes in your diet and/or physical activity or lifestyle may pose any risk to your health. WellOnTrack Services and Programmes are not intended for use by minors or by individuals whose pre-existing health conditions, whether diagnosed or not, make changes to diet, to physical activity and lifestyle unsafe or otherwise inappropriate.

Furthermore, WellOnTrack shall have no obligation or responsibility to monitor users' health statuses or conditions, nor to contact or alert any medical or emergency professional. Your reliance on any information obtained through my site or services is entirely at your own discretion, and WellOnTrack disclaims all liability to you or any other parties in connection with such information.

Privacy

What Data Do I Collect, Keep And Why Do I Need It?

I collect data that you voluntarily provide, including, but not limited to, when you complete a form, enrol in a programme, join a messenger chat group, or otherwise communicate with your health coach. This information may include your names, age, gender, nationality, postal address, phone/contact details, occupation, and any other relevant information that you may provide. This is information that helps me get to know you and to contact and communicate with you.

Session notes - I keep brief notes of our session(s), these will be kept securely. Generally I will destroy/delete all notes after the end of our coach-client contract, and only keep your contact details, unless you indicate that you might at a later stage engage me as your Health Coach again.

Session voice or video recordings – I will always ask for your permission to take recordings. The recordings are for my own use only. They help me in my professional development. All recordings are kept securely on my password-protected mobile phone or PC, and deleted after the session note-taking.

I might also ask you about your current living/family situation and about your medical history including current treatment and medication(s) you take. I'm also interested to learn about which other health professionals and services are supporting you - that helps me to understand you and your health situation better.

WellOnTrack abides by The Code of Health and Disability Services Consumers' Rights ('the Code'), which establishes your rights as a consumer, and the obligations and duties of your Health Coach.

Will I Share Your Data And If I Do, With Whom and What For?

It is very unlikely that I will share your data. WellOnTrack will not sell on or use it for unethical reasons. I may have to share it if my notes are subpoenaed by Court, or if you or anyone you tell me about is at harm or risk of harm, I may have to pass on this information.

I may also discuss your case during my professional supervision, but not using any name. I may use a voice or video recording occasionally for a professional mentor or supervision session that serves my own learning. I will always ask you for your permission before I do so. Supervisors and mentors are bound to a professional code of conduct and strict confidentiality.

How will I store your data?

All notes are stored on my password-protected computer, and any hard copies in a locked filing cabinet, with the key kept securely. Your phone number(s) and recordings may be kept in my password protected mobile phone and/or in my computer. Only I access your information.

How long will I store your data, and how will I dispose of it?

I will keep only your phone and email contacts, the details that are part of the coach-client contract/agreement that you signed, and any data related to financial transactions/information (invoices, payment statements) as required by my insurer and IRD. Any other data I will delete/destroy at the end of the coaching-client contract, unless you wish to extend/renew the coaching contract or you express interest in engaging me as your Health Coach in the future. In that case I will discuss with you which data might be useful to keep.

Website Users

WellOnTrack respects your privacy, and this is important. It will therefore do its utmost to protect it. It will never rent, loan, trade or sell your information. This Privacy policy is here for you to review when you wish. Changes and/or additions will be loaded here. WellOnTrack will only share your information when agreed to by you, and when needed to do so to provide you with the services you sought, or when it is legally required to do so. It will be transparent should a privacy breach occur. I will answer any questions or concerns you have regarding your privacy.

Personal information

WellOnTrack and the website, www.wellontrack.nz, require your personal information so it, or its partners, can provide you with the goods, services or information requested. Your information is kept safe and is not given or on-sold to other parties. It is collected when you contact WellOnTrack. By contacting me, or using the WellOnTrack website, you agree to the collection and use of information in accordance with this policy.

What Personal Information Might WellOnTrack Collect?

The type of information WellOnTrack and its website www.wellontrack.nz collects about you may include information such as: your name, your email address, your address, your phone number(s), other information you may supply, information about your computer via Google Analytics (e.g. your IP address and browser type), information about how you use our website via Google Analytics (e.g. which pages you have viewed, the time you viewed them and what you clicked on), information about your mobile device via Google Analytics (such as your geographical location).

How Long Does WellOnTrack Hold My Information?

WellOnTrack keeps your information for the period necessary to fulfil our obligations to you for the services you require, and as long as it is so obliged by New Zealand law.

Security

WellOnTrack respects your privacy and personal information and takes all reasonable steps to ensure it is kept safe. However no method of transmission over the Internet, or method of electronic storage, is considered 100% secure, therefore should you suspect any loss or misuse of any personal information, please let us know as soon as possible.

Cookies
